When it comes to providing care and support for individuals facing health or social challenges, care assessments play a crucial role in determining the specific needs and requirements of you or your loved one. This comprehensive evaluation is designed to create a tailored care plan that addresses unique circumstances. An assessment is a chance to talk about any issue you are facing and get the personalised assistance you need.

What is a Care Assessment?

A care assessment, also known as a needs assessment or care needs assessment, is a systematic and thorough evaluation that identifies an individual's physical, emotional, and social needs. The goal is to determine the level of support required to enhance the person's overall well-being and quality of life. Care assessments are often conducted by healthcare professionals, social workers, or care coordinators, depending on the specific context and the nature of the individual's needs. You can expect your chosen Care provider to complete this as part of their initial assessment, once your care connection has been made on Pairly.com.

If you would like to find out what your care needs are, you can try our care calculator which helps to determine the amount and types of assistance you or a loved one may need. Our care calculator has 11 multi-choice questions, all designed around UK care assessment standards.
